What speech pathology for older adults actually involves

Speech pathology for older adults goes well beyond helping someone speak more clearly. Our speech pathologist works with older people and their families across Brisbane to manage swallowing safety, communication changes, mealtime risk and voice disorders, at home where the real challenges happen.

Swallowing assessment and dysphagia management

Dysphagia (difficulty swallowing) is common in older adults and carries serious risks including aspiration pneumonia, malnutrition and dehydration. Our speech pathologist assesses swallowing function at home during a normal meal, identifies the level of risk, and recommends strategies such as texture-modified diets, safe swallowing techniques, positioning changes and environmental modifications to reduce aspiration risk.

Mealtime management and safety

Mealtime is about more than swallowing. Our speech pathologist assesses the full mealtime environment: seating and positioning, pace of eating, supervision needs, food preparation, and how carers are supporting the person. For people living with dementia, stroke or Parkinson's disease, safe mealtime management can prevent hospital admissions and maintain nutrition and quality of life.

Communication after stroke or brain injury

Stroke, traumatic brain injury and progressive neurological conditions can affect the ability to speak, understand language, read, write and use numbers. Our speech pathologist assesses the type and severity of communication difficulty, develops a therapy programme tailored to the person's goals, and coaches family members and carers on strategies to support everyday communication.

Communication and dementia

Communication changes are one of the most distressing aspects of dementia for both the person and their family. Our speech pathologist works with the person and their carers to maintain communication for as long as possible, introduce strategies as language skills change, and train carers in how to adapt their communication style to reduce frustration and confusion.

Voice disorders in older adults

Age-related voice changes such as a weak, breathy or strained voice can affect confidence, social participation and the ability to be heard. Our speech pathologist assesses voice function, identifies whether the changes are age-related or indicate an underlying condition that needs medical review, and provides voice therapy exercises and strategies to improve vocal strength and clarity.

Carer training and HCP documentation

Carers play a critical role in implementing speech pathology recommendations, particularly around mealtime safety and communication strategies. Our speech pathologist trains family members and paid carers in safe feeding techniques, communication approaches and when to seek urgent review. We write clear reports for HCP providers, GPs and hospital teams.

Aged care speech pathology in Brisbane: common questions

What can a speech pathologist do for an older person?

A speech pathologist assesses and manages swallowing difficulties (dysphagia), communication changes from stroke, dementia or other neurological conditions, voice disorders and mealtime safety. For older adults, this is often the difference between eating safely at home and repeated hospital admissions for aspiration pneumonia. We visit at home so we can assess the person during real meals and daily routines.

How do I know if my parent has a swallowing problem?

Common signs include coughing or choking during meals, a wet or gurgly voice after eating or drinking, food or liquid leaking from the mouth, taking a long time to finish meals, avoiding certain textures, unexplained weight loss, and recurrent chest infections. If you notice any of these, a speech pathology assessment is recommended. Call your parent's GP or contact us directly.

How is aged care speech pathology funded?

Most clients access our speech pathology through a Home Care Package (HCP Level 1-4), CHSP (Commonwealth Home Support Programme) or DVA (Gold and White Card). Private funding is also available for people not yet in the funded system. If you are unsure what applies, call us and we will help you work it out.

Does my parent need a GP referral?

A GP referral is not always required for HCP, CHSP or DVA-funded speech pathology, but it is recommended and many providers request one. For private clients, no referral is needed. If there is an acute swallowing concern or recent hospital discharge, we recommend speaking with the GP first.

Can the speech pathologist visit at home?

Yes. All our speech pathology sessions for older adults are delivered at home across Brisbane and the north side. Home-based assessment is essential for mealtime management because we need to see the actual eating environment, food preparation, carer support and positioning rather than a simulated clinic setting.

What is a texture-modified diet and who decides?

A texture-modified diet changes the consistency of food and fluids to reduce the risk of choking and aspiration. The speech pathologist assesses swallowing function and recommends the safest texture level using the IDDSI (International Dysphagia Diet Standardisation Initiative) framework. We train carers and family members on how to prepare food and drinks to the recommended consistency.

Can the speech pathologist work alongside the OT or psychologist?

Yes. Astrad is a multidisciplinary allied health practice. Our speech pathologist, occupational therapists and psychologist regularly coordinate care for older clients who need support across multiple areas. For example, an OT might assess kitchen and mealtime positioning while the speech pathologist manages the swallowing and food texture component.
