What aged care OT actually involves in practice

Aged care occupational therapy is not a generic safety checklist. It is a clinical assessment of how an older person is functioning in their own home, what is changing, and what practical supports will extend their independence. Our OTs work across Brisbane and north Brisbane suburbs, visiting at home where we can observe the person in their actual environment.

Home environment assessment

Your OT visits the home and assesses it room by room in the context of how the person actually uses the space. We look at bathroom access, kitchen safety, bedroom setup, entry and exit points, lighting, flooring and outdoor areas. The goal is to identify where the environment is creating risk or limiting independence, and what changes would make the most practical difference.

Falls risk assessment and prevention

Falls are the leading cause of injury-related hospitalisation for older Australians. Our OTs assess both the environmental factors (trip hazards, poor lighting, slippery surfaces, lack of handrails) and the person's functional factors (balance, strength, transfer technique, footwear). We prioritise recommendations by risk level so the most critical changes are addressed first through the HCP or other funding.

Equipment prescription

When an assessment identifies the need for assistive equipment, your OT prescribes the right item and writes the clinical justification required by the funding body. This includes shower chairs, toilet frames, walking aids, bed rails, pressure care products, rise-recliners and kitchen aids. We coordinate with suppliers, arrange trials where appropriate, and follow through to delivery and fitting.

Home modifications

From grab rail installation and improved lighting through to bathroom conversions and ramp builds, our OTs specify what modifications are needed and produce the documentation for HCP, CHSP or DVA funding approval. We coordinate with builders and suppliers, and review the completed work to confirm it meets the person's needs.

Daily routine and self-care support

For many older people, managing daily tasks like bathing, dressing, meal preparation and medication management becomes harder over time. Our OTs assess where routines are breaking down and recommend practical strategies: task simplification, energy management, adaptive techniques and equipment that lets the person do more independently rather than relying on carer support for everything.

Carer and family support

Adult children coordinating care for a parent, often from across Brisbane or interstate, need clear information about what has been assessed, what has been recommended and why. Our OTs write detailed reports for HCP providers and family members, and are available for phone debriefs after the visit. We also coach family carers on safe manual handling, daily routine support and when to escalate concerns.

Your OT visits the home, assesses the person and the environment, and provides clear recommendations. A written report goes to the HCP provider, family and GP as needed. If equipment or modifications are recommended, your OT coordinates the process through the funding.

Aged care OT in Brisbane: common questions

What does an aged care OT actually do?

An occupational therapist assesses how an older person is functioning at home and identifies what practical supports will help them stay safe and independent. This includes assessing falls risk, prescribing equipment (shower chairs, walking aids, bed rails), recommending home modifications (grab rails, ramps, bathroom conversions), supporting daily routines and coaching carers. We visit at home so we can see the real environment, not a clinical setting.

How is aged care OT funded?

Most clients access our OT through a Home Care Package (HCP Level 1-4), CHSP (Commonwealth Home Support Programme) or DVA (Gold and White Card). We also see private clients who are not yet in the funded system or want to supplement their package. If you are unsure what funding is available, call us and we will help you work it out.

My parent has a Home Care Package. How do I arrange an OT?

Talk to your parent's home care provider and request an OT referral, or call us directly. We can advise on how to request the service through the provider. Some HCP providers have waiting lists for OT. If that is the case, call us about private options in the interim.

How soon can we see an OT?

Most new aged care referrals in Brisbane are booked within two to four weeks. We triage urgent referrals such as hospital discharges, safety-critical home modifications and urgent equipment needs. If there is a wait in your area, we will tell you up front.

Does the OT just do one visit, or ongoing support?

It depends on what is needed. A one-off home assessment and report is common for equipment or modification recommendations. For ongoing support such as daily living retraining, falls prevention programmes or progressive conditions, regular visits may be appropriate. Your OT will recommend a plan and your HCP provider can fund it accordingly.

Can the OT help after a hospital discharge?

Yes. Post-hospital OT is one of the most common referrals we receive. Our OTs assess the home before or after discharge, identify what equipment or modifications are needed for a safe return, and coordinate with the hospital discharge team and HCP provider to get things in place quickly.
