OT hand therapy: from assessment to return to function
Hand and upper limb injuries affect almost every task in a day. OT hand therapy works on restoring functional use of the hand through a structured progression from acute management, splinting and scar work, through to graded retraining of the specific activities you need to get back to. Our OTs come to your home or workplace so the rehabilitation happens in the context that matters.
Upper limb functional assessment
Your OT starts with a structured assessment of your hand and upper limb, covering range of motion, grip and pinch strength, sensation, pain behaviour, oedema and the specific functional tasks that have been affected. The assessment establishes a baseline, identifies priorities and produces the initial report required by your insurer, surgeon or treating team.
Splint prescription and fitting
Splinting positions the hand or wrist to protect a repair, support healing, reduce pain or prevent deformity. Your OT assesses whether a custom-moulded or prefabricated splint is appropriate for your stage of recovery and the specific injury. Custom splints are fabricated at your home or workplace using low-temperature thermoplastic material and adjusted to fit. Wearing schedules are provided and reviewed at each visit.
Scar and oedema management
After surgery or significant hand injury, scar tissue and swelling can limit movement and cause ongoing sensitivity or pain if not addressed early. Your OT provides scar massage techniques, desensitisation strategies, compression garment assessment and oedema management in the appropriate clinical stage of recovery, coordinated with your surgeon's post-operative protocol.
Movement and strength retraining
Regaining range of motion and functional strength requires structured, progressive exercise specific to the repair or condition. Your OT prescribes a home exercise programme graded to your recovery stage and reviews it at each session, adjusting based on how the tissue is responding. Exercises are practised in the context of real tasks, not just isolated joint movements.
Return to daily tasks and work activities
The end goal of hand therapy is being able to use your hand for what you need to do. Your OT works with you on the specific daily tasks and work activities that are currently limited, grading the demand as your capacity improves. For workers comp and CTP clients, your OT documents functional progress at each stage and writes the reports required by the insurer.
Coordination with surgeon and treating team
Hand therapy is most effective when the OT works within your surgeon's post-operative protocol and alongside your physio, GP and case manager. With your consent, your OT communicates directly with the treating team, follows surgical timelines for mobilisation and loading, and flags any concerns about healing or progress. This keeps the rehabilitation consistent and reduces the risk of setbacks from miscommunication.
Hand therapy OT is funded through workers compensation (WorkCover QLD, iCare NSW, WorkSafe Vic) for work-related hand injuries, CTP insurance for motor accident injuries, DVA for eligible veterans, and NDIS where an upper limb condition relates to a participant's disability. Private health insurance may also contribute.

What does an OT do in hand therapy?
OTs in hand therapy work on restoring function to the hand and upper limb after injury, surgery or the onset of a condition affecting grip, strength, sensation or movement. This includes wound care and scar management (in the appropriate clinical stage), custom and prefabricated splinting, range-of-motion and strengthening exercises, sensory retraining, graded return to daily and work tasks, and functional retraining for specific activities.
What hand and upper limb conditions do your OTs work with?
Our OTs work with adults managing hand fractures and dislocations, flexor and extensor tendon injuries, nerve injuries (median, ulnar, radial), post-surgical rehabilitation after procedures including tendon repair, carpal tunnel release and joint replacement, carpal tunnel syndrome, De Quervain's tenosynovitis, trigger finger, complex regional pain syndrome (CRPS), crush injuries, and upper limb amputations.
What is the difference between an OT and a physiotherapist for hand therapy?
Both OTs and physiotherapists can provide hand therapy. OTs bring a specific focus on restoring the ability to perform daily tasks and work activities, alongside the clinical hand therapy techniques. OTs also write the functional reports and AT assessments that insurers and NDIS require. Some OTs pursue additional hand therapy training and experience. If you're unsure which is right for you, call us and we'll be straightforward.
What is splinting and will I need it?
Splinting involves making or fitting a custom-moulded or prefabricated device that positions the hand or wrist in a way that supports healing, protects a repair, or reduces pain. Whether splinting is required depends on the type and stage of your injury or condition. Your treating surgeon or GP will often specify splinting requirements, and our OT will assess and fit the splint at your home or workplace.
Can I access hand therapy OT through workers compensation?
Yes, in most cases where the injury is work-related and the claim has been accepted. Workers compensation schemes (WorkCover QLD, iCare NSW, WorkSafe Vic) cover OT hand therapy as part of rehabilitation. Approval depends on your insurer. CTP insurance covers hand therapy for eligible motor accident injuries. We will confirm what documentation your insurer requires and write reports in the appropriate format.
How many sessions will I need?
This depends on the injury, the surgery performed, the stage of recovery and your goals. Some clients need a short series of sessions focused on a specific task or work demand. Others have a longer programme spanning multiple months, particularly after significant reconstruction or when complex regional pain syndrome is involved. Your OT will propose a schedule and review it with you and your treating team as recovery progresses.
Do you provide hand therapy for non-injury conditions like arthritis?
Yes. Our OTs work with adults managing osteoarthritis and rheumatoid arthritis affecting the hands and upper limbs, providing joint protection strategies, assistive equipment, splinting for pain management and support for maintaining independence with daily tasks. Funding for these conditions may come through NDIS, private health insurance, DVA or self-funding depending on your situation.
