An NDIS plan review is when the NDIA reassesses a participant's plan and funding. To support the review, participants and support coordinators often need an OT-written progress report that documents what the person has worked on, how their function has changed, and what supports they will need in the next plan period. What an OT plan review report covers and how to get it done on time

An NDIS plan review report provides the functional evidence a support coordinator or participant needs to go into a plan reassessment with the NDIA. It documents what has changed, what hasn't, what supports are still needed and why, and what the next plan period should fund. Plan review reports are time-sensitive by definition: the review date drives the timeline, and getting the referral in early is the most important thing a support coordinator can do.

Progress report vs new FCA: what's appropriate

A plan review report is usually a progress update against the existing plan, not a new full functional capacity assessment. If the participant's function has changed significantly, if new funding categories are being sought, or if there is no recent FCA on file, a fresh FCA may be more appropriate. We advise on which report type suits the situation when you make contact, so the right document is ready before the review.

What the report documents

The plan review report covers: the participant's current functional capacity; progress made during the current plan period against stated goals; any changes in support needs (increased or reduced); the supports that will be required in the next plan period; and evidence-based recommendations to support the funding request. The NDIA looks for specific, functional evidence, not a general summary of services provided.

Assessment visit or record review

Depending on the participant's situation, the OT either conducts a home assessment visit to observe current function directly, or writes the progress report from existing session records and clinical notes. For participants already receiving ongoing OT from Astrad, the report can usually be written from records. For new referrals, a brief assessment visit ensures the report reflects current function accurately.

Timelines: how far ahead to refer

We generally need two to four weeks from initial contact to deliver a completed plan review report: intake, scheduling, assessment or record review, and report writing. If the review date is already close, contact us immediately and we'll advise whether an expedited turnaround is possible. Support coordinators managing multiple plan reviews can discuss priority scheduling with our intake team.

What the NDIA expects from a plan review report

Plan review reports need to be specific about functional change and support requirements, not just a list of therapy sessions attended. The NDIA expects to see a clear connection between the participant's functional capacity, the supports they've been using, the outcomes those supports have produced, and what is needed going forward. Reports that focus on activity counts rather than functional evidence are the most common reason reviews are queried.

Coordination with the support team

With the participant's consent, we liaise with support coordinators, plan managers and other treating clinicians to ensure the plan review report is consistent with the broader evidence being submitted. The completed report is sent directly to whoever is nominated: the support coordinator, plan manager, the NDIA, or the participant. Follow-up questions from the NDIA are answered at no extra charge.

Assessment visit or record review

Depending on the participant's situation and the purpose of the report, the OT may conduct a home assessment visit to observe current function, or write the progress report based on session notes and records from ongoing therapy. We'll confirm the most appropriate approach at intake.

Report delivered ahead of your review date

The completed plan review report is delivered digitally within two to three weeks of the assessment or record review. We can send it directly to the support coordinator, plan manager or the participant. If there's an urgent review deadline, let us know at intake.

What is an NDIS plan review report?

An NDIS plan review report is an OT-written document that supports a participant's plan reassessment with the NDIA. It typically documents the participant's current functional capacity, the supports they have been receiving, the progress made towards their goals, any changes in their support needs, and recommendations for the next plan period. The NDIA uses this evidence to inform funding decisions for the next plan.

Is an OT plan review report the same as a functional capacity assessment?

Not always. A full functional capacity assessment (FCA) is a comprehensive baseline document, while a plan review report may be a shorter progress update that references the existing FCA. If the participant's function has changed significantly or if significant new funding is being sought, a new FCA may be more appropriate. We'll advise on the most suitable report type when you make contact.

When should I request the plan review report?

As early as possible before the review date. We generally need two to four weeks from initial contact to deliver a completed report: time for intake, scheduling the assessment visit, assessment, and report writing. If you know your participant's review date, contact us as soon as it's confirmed. Urgent timelines may be possible depending on availability.

Can a support coordinator request the report on behalf of the participant?

Yes. We regularly receive plan review referrals from support coordinators and plan managers. We keep the support coordinator informed throughout the process and send the completed report to whoever is nominated. Participant consent is confirmed at intake.

What if the participant hasn't been receiving OT from Astrad?

That's fine. We can conduct a fresh assessment visit and write the plan review report based on that, even if the participant has been receiving OT from another provider. We'll ask for any existing reports or records to inform the assessment. Alternatively, if you have an existing OT provider whose records are relevant, let us know and we'll work out the best approach.

What funding covers an OT plan review report?

OT plan review reports are typically funded from the Capacity Building budget (Improved Daily Living support category) or, in some cases, Core Supports, within the NDIS Pricing Arrangements and Price Limits. We provide a written quote before any work is started. Contact us for an estimate based on your participant's situation.

Do you work with agency-managed participants?

Astrad works with self-managed and plan-managed NDIS participants. If a participant is agency-managed, their support coordinator can explore options including a move to plan management. We're happy to discuss this with you.
