How OT supports children with developmental delay

Developmental delay is not a single thing. It describes a child who is reaching motor, cognitive, communication or self-care milestones later than expected across one or more areas. OT focuses on the functional gaps: what the child cannot yet do independently in their daily life, and what is driving that gap. The programme is built around current priorities and updated as the child develops, not locked to a fixed protocol.

Developmental assessment across domains

Your OT assesses your child's fine motor skills, gross motor foundations, sensory processing, self-care abilities and play skills using standardised developmental assessments. This gives a clear picture of which areas are delayed, by how much, and what the functional consequences are in your child's daily environments. The written report provides a baseline for measuring progress over time.

Early intervention and skill building

For children under five, early intervention builds skills during the period when the brain is most responsive to input. Your OT works on the specific developmental skills your child needs right now: hand function, feeding, sensory regulation, play and the foundations for school readiness, using play-based activities that are motivating and developmentally appropriate.

Fine motor skill development

Fine motor delays affect everyday tasks: feeding, dressing, drawing, play and eventually writing. Your OT assesses hand strength, coordination, grasp patterns and in-hand manipulation, then builds a structured programme that develops these skills progressively, always connecting therapy activities to the real tasks your child needs to manage at home and at childcare.

Gross motor foundations

Balance, body awareness and coordination often underpin difficulties with sitting, playing and classroom participation. Your OT assesses gross motor development, identifies where the gaps are in core stability, postural control and coordination, and works alongside physiotherapy where both are relevant to ensure your child's motor development is supported across all levels.

Self-care skill progression

Mealtimes, dressing, toileting and personal hygiene are daily activities where developmental delay creates real, practical challenges for families. Your OT breaks each self-care task into component steps, identifies the specific barriers (motor, sensory or sequencing), and builds graded practice into daily routines at home, making independence achievable in small, consistent steps.

Family and educator coaching

Skills built in OT sessions transfer most effectively when families and educators know how to support them consistently throughout the day. Your OT provides parents and childcare or school staff with specific strategies, home programmes and practical modifications: how to set up the environment, how to prompt without taking over, and how to build practice into everyday routines.

What does OT cover for a child with developmental delay?

OT for developmental delay typically focuses on fine motor skills (hand function, drawing, self-care tasks), gross motor foundations (balance, coordination, body awareness), sensory processing, self-care (dressing, feeding, toileting), play skills, and pre-school or school readiness. The specific focus depends on the child's age, goals and what the assessment finds.

My child has a global developmental delay diagnosis. Does that change the OT approach?

A global developmental delay diagnosis tells us that multiple areas are affected. The OT approach does not change fundamentally: we still assess the specific functional gaps and build goals around what is most relevant to the child's daily life right now. The plan is updated as the child develops. Diagnosis informs the picture but the goals remain individual.

How early can you work with a child showing signs of developmental delay?

From around 12 to 18 months, OT can be useful where motor or sensory differences are affecting feeding, play or early self-care. The earlier practical strategies are in place for families, the more time there is to build skills before formal schooling. We support children from toddlers through to teenagers.

Do you work with children who have developmental delay alongside another diagnosis (autism, cerebral palsy, Down syndrome)?

Yes. Many children we work with have a combination of diagnoses or presentations. Our OTs are experienced in working with children with a range of co-occurring conditions and tailor the approach accordingly. We coordinate with the wider support team, including speech pathology, physiotherapy and paediatrics, where relevant.

Does NDIS fund OT for developmental delay?

Yes. Children with a developmental delay diagnosis may be eligible for NDIS funding, which covers OT under the Improved Daily Living (Capacity Building) budget. Early Childhood Approach (ECA) funding is also available for children under 9 with developmental concerns even before a formal diagnosis is confirmed. Our team can walk you through the options.

We are waiting for a formal diagnosis. Can OT start in the meantime?

Yes. You do not need a diagnosis to access OT. If your child has functional difficulties affecting daily life, OT can begin while diagnostic assessments are underway. In fact, OT reports can contribute useful information to the diagnostic process.

What does a developmental delay OT assessment look like?

Our OT visits your home and, where relevant, your child's childcare or school setting. They observe your child completing age-appropriate tasks, use standardised developmental assessments, speak with parents and caregivers, and produce a written report with a recommended therapy plan. The first session is low-pressure and child-led.
